Welcome to the Office of Health Services!  In addition to nursing services provided at all schools, Health Services administers a wide variety of programs to serve the health and wellness needs of students and families.  Our office is located in-between the Liberty Elementary and Woodcrest Junior High School campuses, off Bon View Avenue, South Ontario.  You can call us at (909) 628-1201, extension 6769, or fax (909) 548-6090.

"FRIEND TO FRIEND THANKSGIVING DINNER PROGRAM"
The C.A.R.E. Program is presenting an opportunity to extend the Thanksgiving tradition to those families in need.  Beginning November 2, 2009 you my purchase a Thanksgiving dinner under the name of the C.A.R.E. Program at any one of the participating grocery stores listed below.  Please have the store charge you on seperate transactions if you wish to purchase more than one dinner.  Dinners starting at $45.99 & up... Please send your receipt to Health Services/CARE Program.

California School Immunization Law requires children be up-to-date on immunizations to attend school or child care. Most children need booster shots before starting kindergarten. Seventh grade entry requirements went into effect July 1, 1999.

California Education Code, Section 49452.8, now requires that your child have an oral health assessment (dental check-up) by May 31 in either kindergarten or first grade, whichever is his or her first year in public school.
