Welcome to the Office of Health Services!  In addition to nursing services provided at all schools, Health Services administers a wide variety of programs to serve the health and wellness needs of students and families.  Our office is located in-between the Liberty Elementary and Woodcrest Junior High School campuses, off Bon View Avenue, South Ontario.  You can call us at (909) 628-1201, extension 6769, or fax (909) 548-6090.

The C.A.R.E. and TYKES Programs have moved to a new office.  They are now located at the Adult School Campus (12970 Third Street).  TYKES new extension is 8960 and for C.A.R.E. please use extension 8967 or 8968.

 CHILDHOOD IMMUNIZATIONS
California School Immunization Law requires children be up-to-date on immunizations to attend school or child care. Most children need booster shots before starting kindergarten. Seventh grade entry requirements went into effect July 1, 1999.

California Education Code, Section 49452.8, now requires that your child have an oral health assessment (dental check-up) by May 31 in either kindergarten or first grade, whichever is his or her first year in public school.
