Welcome to the Office of Health Services!  In addition to nursing services provided at all schools, Health Services administers a wide variety of programs to serve the health and wellness needs of students and families.  Our office is located in-between the Liberty Elementary and Woodcrest Junior High School campuses, off Bon View Avenue, South Ontario.  You can call us at (909) 628-1201, extension 6769, or fax (909) 548-6090.

California is experiencing an epidemic of whooping cough. In response, the Department of Public Health has implemented a new Pertussis/Whooping cough vaccine (Tdap) booster requirement for students entering grades 7-12, beginning July 1, 2011.
